-- flap Sx. after 2 months, still bone graft and poor bone formation
-- remove bone graft, note the buccal wall is intact
-- re-GBR Sx. (FDBA + collagen membrane ) 2012.8.24
-- F/U 6 months, then implant Sx.
-- implant Sx. () 2013.3.1
