lower anterior tooth area GBR

lower 11 21 severe perio destruction, post-EXT 2 months
-- decorticatino of lower bone
-- Ti-mesh, collagen mem, FDBA 0.5 cc
-- 5-0 nylon suture

-- try to think question
   A. pre-OP CT to confirm bone shape and quality
   B. tending screw or other space making tool
   C. new collagen membrane (Taiwan company pig dermis ), hard to control
   D. suture technique and material
   E. mini implant ??

2013/09/16
implant surgery visit (posterior GBR 6 months)
-- remove Ti-mesh and and periosteum-like tissue attache Ti-mesh
-- about 3-4 mm horizontal bone regain after GBR (original 1-2mm bone horizontal bone width )
-- few residual bone graft particle above ridge
-- mini implant is selected to implant (MDL 2.5 mm x 13mm length )
-- suggest single crown and resin to correct anterior contour

lower posterior ridge GBR and implant

Case :
-- 36 37 EXT s/p b-TCP + HA + colla-tape for socekt preservation
-- flap Sx. after 2 months, still bone graft and poor bone formation
-- remove bone graft, note the buccal wall is intact
-- re-GBR Sx. (FDBA + collagen membrane ) 2012.8.24
-- F/U 6 months, then implant Sx.
-- implant Sx. () 2013.3.1

真糟糕爛透了的結果

第一次嘗試的immediate implant

一步一步都小心遵照著principle 去做

怎知變化永遠比計畫來的大

每一隻都或多或少有buccal dishesience

尤其是在美觀區

查了一些資料

大概只能做 esthetic soft tissue graft 的處理了

shit

壓力大的CASE

Severe chronic periodonitis, 預估能留的有 34 35 37 45 其它 all extraction

P't 不考慮CD or RPD 甚至 implant support denture 也不在列只願意 implant FPD

思考的是 : 時間 GBR的順序 immediate implant的可能性 interim denture 的佩帶

好多好多的思考
